Are you an experienced GP looking to build a portfolio career or just looking for a change of direction? Do you like the idea of working in an experienced multidisciplinary team? We are looking to appoint a part-time GPwSI in Rehabilitation and Older Person’s Care who will provide clinical support and leadership to a multidisciplinary team at Skylark Rehabilitation Unit and will contribute to the service maintaining a reputation for clinical excellence.
The team will work to ensure that service users receive the right care in the right place at the right time. You will have the ability to support others from a variety of disciplines to develop their skills in advance care planning, medication reviews and frailty syndromes. We hope you will also be able to develop the prognostic skills to support advanced care planning where appropriate.
You will develop strong links with GPs and community teams as well as secondary care, both to improve care pathways and effectively communicate decisions about individual patients.
As a GPwSI you will be required to ensure our services are safe, effective and caring, and bring clinical leadership and a passion for excellence, responding to the needs of our service users, carers and their families and our local communities.
