Practice Nurse – Mendip Country Practice

Would you like a consulting room with a view, rolling hills and beautiful sunsets, then look no further. Mendip Country Practice is looking to recruit an experienced and motivated Chronic Disease Nurse to join our small friendly nursing team.

The post holder will provide and maintain a high standard of chronic disease management for patients with chronic diseases, which includes the diagnosis, monitoring and development of patient’s individual management plans and agreeing these as appropriate with the patient and other health professionals.

The post is for a minimum of 24 hours per week, but we are willing to negotiate.

Salary – dependant on experience

Main duties of the job

The Successful candidate will work as part of the primary healthcare team providing General Medical Services to the patients of the Practice. Their main responsibility will be to provide for chronic disease management of our patients.

The successful applicant must have excellent communication skills and flexibility is imperative due to the nature of the role

The practice is a long established busy rural dispensing practice with 5,600 patients. We have 4 partners, 1 advanced nurse practitioner, 1 practice nurse, 2 HCA, ably assisted by friendly supportive team of admin and receptionists. We are forward thinking, achieving high QOF achievements. We are a training practice for ST Doctors, Medical Students and nurses. We are affiliated to Bristol University and for several years running have won the ‘best medical school teaching practice in Bath’ award. You would have the opportunity to develop your mentoring skills of nursing team members who are undertaking further qualifications. The successful candidate would be actively encouraged to increase their skill set if they so desired, for example training in sexual health, coils and implants, or non-clinical prescribing qualification.
