Diabetes Practice Nurse – Greenway Community Practice

Overview

Position: Diabetes Practice Nurse (Part-Time, 16-20 hours per week)
Greenway Community Practice | NHS Pension | 27 days holiday pro rata

We are seeking a motivated and compassionate Diabetes Practice Nurse to join our friendly and supportive general practice team. This role will involve running structured diabetes reviews alongside delivering routine treatment room nursing duties.

This is an excellent opportunity for a nurse with experience or a strong interest in diabetes care who is keen to develop their skills within general practice.

The successful candidate will:

  • Undertake structured diabetes reviews for patients with Type 1, Type 2 and pre-diabetes in line with NICE guidelines
  • Provide patient-centred education and lifestyle advice to support self-management
  • Support diabetes medication reviews and escalation of care where appropriate
  • Deliver treatment room duties including wound care, injections, immunisations, venepuncture and other routine procedures
  • Work collaboratively with GPs, nurses and the wider multidisciplinary team
  • Maintain accurate clinical records and contribute to QOF and audit requirements

 

We are looking for a nurse who:

  • Is a Registered Nurse with current NMC registration
  • Has experience in primary care and chronic disease management
  • Is confident undertaking treatment room duties
  • Has diabetes training or experience
  • Has excellent communication skills and a patient-focused approach
  • Works well independently and as part of a team

You would be part of a strong and experienced clinical team comprising of 5 GP partners, 5 salaried GP’s, 2 clinical pharmacists, 4 Nurses and 4 HCAs a Paramedic Practitioner and a First Contact Physiotherapist.

We are very proud to have been rated as ‘Outstanding’ by the Care Quality Commission and voted 4 times the ‘Best GP Practice in Bristol’ by our patients in the annual GP patient survey.

We are ranked 65 in the top 100 practices in England and have one of the best patient recommendation scores with 95% of patients recommending our service.

We are a popular training practice with an excellent staff retention rate. We engage in regular clinical meetings and invest time to interact with each other on a daily basis. There is also a mentorship scheme in place for newly qualified GPs.
