Monday – Friday, 30 – 35 hours per week

Tyntesfield Medical Group is seeking an exceptional individual to lead and support our Social Prescribing Team.  This is an exciting opportunity for someone who is passionate about person-centred care, community wellbeing and making a real difference to people's lives.

As Social Prescriber Team Leader, you will play a key role in proactively identifying and supporting patients, particularly those who are frail, elderly, vulnerable or living with long-term health conditions.  You will help individuals improve their health and wellbeing by connecting them with local community groups, activities, services and support networks, enabling them to take greater control of their health and live more independently.

Alongside managing your own caseload, you will provide day-to-day leadership, support and guidance to the Social Prescribing Team, helping to ensure a high-quality, responsive and compassionate service for patients across the practice.
