* QOF and LTC ManagementQOF and LTC Management

Time: 11:00 am

QOF and LTC management

Description:

Long Term Conditions Management solution within Ardens which allows you to identify Patients who require a review based on diagnosis codes added to their records. These patients are then split up by their month of birth as an effective way of spacing the reviews out over the year. The system then facilitates all condition reviews being scheduled at once whether this is one appointment or a series of appointments to cover everything based on how you would like to work as a Practice.

We appreciate that changing a recall system is a significant undertaking for Practices, therefore we are happy to provide a detailed overview of how the system works. We recommend a two hour overview session for this where key members of your teams involved in both the administration and the clinical processes are present. This allows for all of the necessary discussions to take place around how best to implement this in Practice. The system is designed to be somewhat flexible to accommodate differed ways of working so having everyone present for the session is of real benefit.
