Role Overview

• Hours: 15-18 hours per week.

Key Responsibilities

• Lead and deliver chronic disease management clinics following national and local guidance.
• Conduct structured reviews for diabetes and respiratory patients.
• Develop and adjust personalized care plans, including medication changes.
• Promote lifestyle advice and patient self-management.
• Collaborate with the wider clinical and administrative team while maintaining accurate documentation.

Candidate Requirements

Essential Criteria:

• Registered Nurse or Pharmacist with relevant UK registration.
• Independent Prescribing qualification.
• Proven experience in chronic disease management, especially diabetes and respiratory conditions.
• Strong clinical assessment skills and a patient-centred approach.
• Excellent communication and teamwork abilities.

Desirable Criteria:

• Previous experience in General Practice or primary care.
• Treatment room skills, such as wound care (for nursing candidates).
