The Clinical Practitioner will provide exemplary care for their patients, priding themselves on meeting population health needs. This will include consultation with patients who have reached a range of dispositions from 111 including primary care (1 ,2 -,6 -,12- and 24-hour timescales), 999, Emergency Department and Emergency Treatment Centre as well as those who present to practices with routine or urgent needs. BrisDoc practice services specialise in providing high quality and accessible health care to vulnerable, marginalised and excluded groups. Clinicians will need to be focused on improving health inclusion and removing barriers to holistic healthcare.
Main duties of the job
Who We're Looking For
- Master's degree in Pharmacy or equivalent.
- Registered with the General Pharmaceutical Council (GPhC).
- Experience of working in a patient facing role in clinical pharmacy or pharmacy setting
- Strong knowledge of medicines optimisation, polypharmacy and prescribing safety
- It is desirable to have an independent prescribing qualification or working towards one
- Experience of working with I.T systems Microsoft Office and Clinical Systems
- Experience of following set procedures and protocols and working to quality standards
- Experience of working in a busy environment and meeting deadlines

In this role, you will carry out structured medication reviews for patients with complex medication needs, including polypharmacy and frailty, and provide medicines optimisation and reconciliation following hospital discharge and other transitions of care. Working within your scope of practice and under appropriate clinical supervision, you will support repeat prescribing processes, respond to patient medication queries, contribute to clinical audits, and help identify and manage prescribing risks in line with MHRA guidance and national safety alerts.
You will also act as a source of clinical supervision for pharmacy technicians and work alongside prescribing administration colleagues to support safe and effective prescribing.
