I don’t know about you, but I am delighted about the end of the school holidays. Don’t get me wrong, it was lovely to spend time with my kids on holiday, but now I just want my evenings (and house) back.

This is my last push to you all to ensure we have representation from every single practice for the LMC Members’ event next Tuesday 10th September. For ease, I am sharing the registration link within this newsletter, but we only have the capacity for a maximum of two representatives from each practice (Larger multi-practice GP Partnerships have can have two representatives from each of their practices to reflect the need for their larger voice) This is a really important meeting for us to discuss how to act collectively within BNSSG in order to tackle the workload and funding challenges within general practice.

This week I completed my first Medical Examiner process which was very fortunate as this process is to be mandated from 9th September. Although it takes about half an hour to consolidate all the relevant information, it will save time when completing the MCCD. The information shared within Remedy and the Dummies guide (embedded below) was helpful to assist me through this process. Eventually, we will also have electronic MCCDs, and not a moment too soon in my opinion, because I find paper so wasteful and antiquated.

The Medical Examiner System will become mandatory from the 9th September and you will no longer be able to write a death certificate without going through this process.

We appreciate your feedback following the Lead Nurse Community of Practice meeting and the subsequent communications regarding prescribing processes. In hindsight, we realise we underestimated the impact of raising this issue at that time and have reflected on it as a team. Whilst we still believe it was essential to highlight the issues we identified; we recognise that it may have been more effective to explore potential solutions first.

Since the meeting, progress has been made in addressing the raised concerns, and exploring ways that practices may implement these changes safely. We have collaborated with partners across the system to develop a Prescribing Processes SOP, which aims to provide examples of how practices may implement safe working practices. The first draft is complete, and we are awaiting further feedback before sharing it with a broader audience for review. There is no current timeframe for completion; however, we are treating this as a priority and will endeavour to share it with practices as soon as possible.

Question: Please can you clarify the exact rules around onward referrals from secondary care? Specifically, what are the rules around other health care professionals, such as physiotherapists and dieticians?

Answer: For secondary care professionals (including non-medical health care professionals), this is covered by the NHS Standard Contract of 2024/25 as follows:

  • SC 8.2: Responsibility for urgent care must not be handed back to the GP but delivered by Provider if within their scope
  • SC 8.3: Responsibility for urgent care needs collaboration with the GP if it is outside the scope of the Provider and Provider must consider other local services to refer in to
  • SC 8.4: Responsibility for onward referral for same non-urgent condition lies with Provider
  • SC 8.5: Permission of GP required by Provider before onward referral for a different condition

Health care professionals working within the community are not covered by the same contract.

GPC England has taken its own legal advice and confirmed that practices cannot decline this request from NHS England as doing so will risk breaching their contract.
